The Blues are keen to bolster their attacking options before the transfer window closes on Friday, September 1.
The 21-year-old, who is also target for West Ham, has struggled for minutes under Pep Guardiola, starting just twice in the Premier League.
Both appearances came after City had wrapped up the title in 2022/23.
Palmer worked under Chelsea’s co-director of recruitment and talent Joe Shields at City.
Palmer has made a bright start to the new campaign, netting in both the Community Shield and UEFA Super Cup for last season’s treble winners.
But City are open to offers for the England Under 21 European Championship winner.
His chances of regular action look to have decreased further following the arrival of Jeremy Doku from Rennes.
Palmer is not the only forward Chelsea have on their list, with Brentford’s Ivan Toney since emerging as a shock target.

Barcelona‘s Ansu Fati, who is also on Spurs’ radar, along with teammates Raphinha and Ferran Torres are other options.
There also been interest in Arsenal’s Emile Smith Rowe and Brennan Johnson of Nottingham Forest, the latter who is wanted by both Brentford and Tottenham.
Lyon’s 18-year-old winger Bradley Barcola, was a one-time target, but the highly-rated teenager looks set for Paris Saint-Germain.
